Summary
Ziel: Das Ziel dieser Studie war es, die Wirksamkeit des Mistelextraktes Iscador® P beim Equinen Sarkoid im Vergleich zur Placebo‐Kontrolle zu evaluieren.
Methoden: Zwischen Mai 2004 und Dezember 2005 wurden insgesamt 53 Pferde mit 444 Sarkoiden aufgenommen und per Zufall (2:1) einer Behandlungsgruppe (viscum album Extrakt; VAE n=32) oder einer Kontrollgruppe (Placebo n=21) zugeordnet. Die VAE‐Gruppe wurde mit subkutanen Injektionen von 0.1mg bis 20mg/ml Iscador® P (viscum album ssp. Austriacus; Weleda, CH‐Arlesheim) dreimal pro Woche über 105 Tage behandelt. Die Placebo‐Gruppe erhielt physiologische NaCl‐Lösung. Ein Jahr nach Therapiebeginn wurde der Status der Tiere entweder als "geheilt", "gebessert, "stabil" oder "verschlechtert" beurteilt. Im März 2011, somit min. 5 Jahre nach Therapiebeginn wurden die Pferdebesitzer telefonisch interviewt, um langfristige Behandlungsergebnisse bewerten zu können.
Ergebnisse: Die Ein‐Jahres‐Heilungsrate war 28% (9/32) in der VAE‐Gruppe und 14% (3/21) in der Placebo‐Gruppe (n.s.). Darüber hinaus zeigten 4 Pferde der VAE‐Gruppe und 0 Pferde in der Placebo‐Gruppe Verbesserungen. Ein positives Outcome war also bei 41% der Pferde in der VAE‐Gruppe und bei 14% in der Placebogruppe festzustellen (p <0,05). Im Jahr 2011 konnten Daten von 30 VAE‐Pferden und 20 Placebopferden gewonnen werden, von denen konnten 27 (VAE) bzw. 15 (Placebo) ausgewertet werden. Alle primär geheilten oder gebesserten, noch lebenden Pferde (11) der VAE‐Gruppe blieben ohne weitere Behandlung geheilt. Vorher als "stabil" beurteilte Pferde wurden in 6/8 Fällen ohne weitere Behandlung geheilt. Ausschliesslich mit VAE wurden bis 2011 63% Pferde geheilt (17/27), hingegen 4 von 15 nur mit Placebo (27%; p<0.05).
Fazit: Die Behandlung mit Iscador® P stellt eine wirksame Therapie bei Pferden mit Equinem Sarkoid dar. Eine vollständige Heilung entwickelt sich in vielen Fällen über eine lange Zeit, ist dann aber in aller Regel nachhaltig.
Summary translation
Purpose: The purpose of this trial was to evaluate the effects of Iscador® P therapy in comparison to placebo control in horses with Equine Sarcoids.
Methods: From May 2004 to December 2005, a number of 53 horses with 444 sarcoids had been included in the study. After randomization, a treatment group (viscum album extract; VAE n=32) and a control group (Placebo n=21) was established. VAE group was treated with subcutaneously applied Iscador®P (0.1mg to 20mg/ml viscum album ssp. austriacus; Weleda, CH‐Arlesheim) three times a week over 105 days. Placebo group received saline solution.
Outcome after one year was assessed as “cured”, “improved”, “stable” or “deteriorated”. In March 2011 horse owner had been interviewed again in order to evaluate long‐term outcome of the treatment. Results: The One‐year cure rate was 9/32 (28%) in the VAE group and 3/21 in placebo group (14%).
Additionally, 4 horses of VAE group and 0 horses in placebo group showed improvement. The overall positive response was therefore 41% in VAE group and 14% in placebo group (p<0.05). Follow‐up data of 30 VAE and 20 placebo horses could be obtained in 2011. Of these 27 (VAE) and 15 (Placebo), respectively, were analysed. Primarily cured or improved, living horses (11) in VAE group were cured without additional treatment. Formerly “stable” horses (8) were cured in 6 cases without additional treatment. The 5‐years cure rate in the VAE‐group finally was 17/27 (63%) and 4/15 (27%) in the placebo group p<0.05).
Conclusion: Iscador® P is an effective therapy in horses suffering from Equine Sarcoids. Complete cure develops over a long time in many cases, however is of sustainable character.
